Position Summary

The Licensed Practical Nurse (LPN) utilizes skills and knowledge to provide care for our clients in multiple program areas. This role is a part of the integrated Health Services team and is expected to work in both our Urban Response programs and our Recovery Oriented System of Care programs. The Licensed Practical Nurse may perform tasks related to emergency response, applying various standardized assessment tools, medication administration, symptom managements, documentation, health promotion, and other tasks, as assigned by a Team Lead.

Time Commitment

Being a member of the LPN Relief Pool is vital to ensure staffing schedule requirements are met to achieve successful operations of Emergency Shelter and Health Services departments.

In order to remain a key member of the LPN Relief Pool, you must secure a minimum of 3 shifts per month.

You must be available for a minimum of 3 shifts per month in total. This includes unplanned absences of regular, Full-Time LPN teammates (short notice shift call-outs).

If you are unable to work for longer than a 4 week period, you will be removed from the relief list and our employment relationship will end. You may reapply with the agency at a later date when you have more availability.

Job Duties and Responsibilities

  • Perform delegated independent nursing functions within the scope of practice as determined by CLPNA and the DI within the individual LPN competence level, as observed by the Nursing Team Lead(s) and/or other Clinical Leadership
  • Assist with Adult Care Worker or Recovery Care Worker duties, when time allows
  • Consistently demonstrate the ability to take the initiative to make decision/choices without direct supervision
  • Be informed of APL routines and maintains standards and knowledge around phlebotomy and lab services
  • Participate in formal and informal health teaching including preventative health concepts concerning Hepatitis A, B, C and AIDS, condom usage, nutrition, STBBI’s, basic sanitation, personal hygiene, post-op, and pre-natal care
  • Participate in influenza and other vaccination clinics
  • Maintains and ensures best practices around the handling of vaccines, participates in the maintenance of immunization procedures and administration of vaccine
  • Ensure compliance with regulatory standards (SDS, OH&S, WHMIS)
  • Organize job functions and work area to be able to effectively complete assignments within established time frames
  • Provide culturally aware support to DI clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present
  • Ensure all required data is entered accurately into organizational databases including the DI Guestbook Database, and client Electronic Medical Records (EMR) and referrals provided in accordance to DI policy and professional nursing standards
  • Develop and maintain collaborative and accountable working relations with internal DI colleagues to strengthen communication flow and cross-program understanding
  • Provide caring, reliable, trauma-informed care to all patients & clients accessing the DI to reduce barriers to service and increase community trust
  • Conduct assessments, initiatives, and implement health management strategies; provide appropriate evaluation, follow-up, and modifications to care plans according to practice protocols; assist with the delivery of disease management and health promotion; implement strategies for improving the quality and provision of care
  • Coordinate services with other team members and community resources according to policies, procedures, and established standards
  • Perform other related tasks that are approved by the designated Nursing Coordinator, Senior Manager, Health Services, and the Directors of the DI

Required Competencies and Qualifications

  • Current registration as an LPN with the CLPNA and proven experience as an LPN
  • Ability to work in a diverse care setting with a multidisciplinary team
  • Displays compassion always, with a commitment to standards and process
  • Completion of a university, college or other approved Licensed Practical Nurse program is required
  • Experience in acute care is an asset
  • Advanced Wound Care experience is an asset
  • Mental Health experience is an asset
  • Current CPR - HCP certification is required
  • Energetic and self-directed, with effective time management and organizational skills including the ability to deal with several priorities at the same time
  • Strong strategically focused analytical skills, good common sense; and capacity to provide leadership
  • Well-developed inter-personal and conflict resolution and negotiating skills
  • Excellent communications skills, both oral and written
  • Computer skills including familiarity with word processing, database, and Internet

Working Conditions

Front-line staff are exposed to emotionally impactful events, such as: medical distress, threatened/actual serious injury, violence, abuse, and death; where clients may be experiencing fear, terror, hopelessness, rage, self-harm and conflict. It is understood and accepted that you will be working in distress and/or high-stress situations, which may include emergency response where resiliency and stress tolerance and the ability to maintain composure, remain calm and focused is imperative. Exposure to bodily fluids may also occur.
