Quality Improvement Coordinator - Registered Nurse

Quality RN Position Summary:

Under joint direction of the Care Guide Partners PACE Medical Director and Executive Director, the Quality Improvement Coordinator develops, coordinates, implements and oversees quality assessment and performance improvement activities for the CARE Guide Partners PACE quality improvement program. The position reviews and monitors the utilization of PACE services, caregiver and participant satisfaction, effectiveness and safety of staff-provided and contracted services as well as monitoring quality improvement for non-clinical areas.

Quality RN Position Duties & Responsibilities:

• Conducts annual and ad hoc reviews of the quality improvement program for Care Guide Partners PACE and updates plan as appropriate. Reports outcome measures related to quality improvement activities and performance.
• Coordinates the development and implementation of program-wide and center-specific quality indicators related to services, satisfaction, effectiveness and safety.
• Monitors quality improvement initiatives for non-clinical areas including but not limited to grievances and appeals, transportation services, meals, life safety and environmental issues.
• Oversees data management for QI activities including the coordination of collection, entry,  analysis and assessment of data.
• Coordinates with national initiatives in Care Guide Partners PACE quality management including development of the accreditation program, CMS’ QI program, National Care Guide Partners PACE Association quality initiatives and technical assistance for developing sites.
• Partners with the Hosparus Health Education Department to ensure indicated education and training opportunities for Care Guide Partners PACE staff and contractors on QI activities and techniques.
• Assists in interface between participants, caregivers and program management for quality-related complaints.
• Attends and supports the Quality Improvement Committee of the Care Guide Partners PACE Board of Directors on Care Guide Partners PACE-related matters.
• Other projects and duties as assigned. 

Quality RN Position Qualifications:

• Licensed RN in the state of Kentucky.
• Minimum two years experience working in healthcare quality improvement. CPHQ certification preferred.
• Minimum 1 year of experience working with a frail or elderly population or must receive appropriate training upon hire.
• Must maintain CPR certification.
• Able to work independently and as an effective member of a team.
• Excellent communication skills, with ability to communicate with internal stakeholders at all levels of the organization and external stakeholders from varied backgrounds.
• Strong analytical skills, with proficiency in data, reporting, and technology-based tools. Able to identify pertinent policy issues and develop strategies, solutions and procedures.
• Excellent organizational skills; able to multitask and maintain numerous initiatives with time sensitivity.
• Working knowledge of principles and practices of healthcare, healthcare systems, managed care, Medicare and Medicaid benefits and regulations.
